• A Certificate of Liability Insurance with an endorsement to Atrevete Collaborative Kitchen Company.

• A Shared Kitchen Food Permit from Kane County, Illinois: https://kanehealth.com/

1. Visit "Permits and Fees" for this form and any other Kane County forms you may require.

2. We've prepared an information sheet to assist you in completing this form.

• Once you're part of our kitchen, we'll need a payment method, such as a credit card or ACH bank information.
